What is PHI?
The Prostate Health Index (PHI) test can be used to aid in evaluating a person's risk of clinically significant prostate cancer. In some cases, it may help to avoid the need for a biopsy in someone with elevated PSA levels. The Prostate Health Index or PHI score is calculated from the results of 3 blood tests: prostate-specific antigen (PSA), Free PSA, and p2PSA. The PHI score provides a more accurate assessment of an individual's risk of clinically significant prostate cancer, but not their likelihood of developing prostate cancer. Besides cancer, conditions that can elevate PSA include aging, infection or inflammation of the prostate gland, enlarged prostate, or benign prostatic hyperplasia.
What does the PHI test include?
This test begins with an initial PSA blood level. If the PSA is within the normal range, no additional testing will be performed.
If PSA is outside of the normal range, PSA Free, % Free PSA, and p2PSA will be performed at no additional cost. In addition, the Prostate Health Index Score will be provided along with interpretive information on the probability of cancer and confidence interval. This testing will only be performed if PSA levels are out of range.
Who should order a PHI test?
PHI is most often ordered by people age 50 or older, especially if they have had elevated PSA levels or other risk factors such as a family history of prostate cancer. While the PSA test has been historically used as a screening tool for prostate cancer, it has been shown to have a limited specificity. This has led to unnecessary concern for individuals whose PSA is elevated by conditions other than prostate cancer. The PHI test has a greater level of accuracy and specificity than other types of blood tests.
Why order a PHI test?
The PHI test can minimize the occurrence of false positives, which can in turn reduce the need for unnecessary biopsies, which can be costly, invasive, and uncomfortable. The PHI test can help aid in the early identification of prostate cancer, which can lead to timely intervention and a better prognosis. It is important to note that the PHI test can neither diagnose nor rule out prostate cancer by itself. The results of this test should be reviewed by your doctor to determine the appropriate follow-up.
